Beyond Survival: Why access to safe blood is a fundamental right
For millions living with chronic blood conditions in India, finding blood is only half the battle. Ensuring that blood is safe from hidden infections is where the real struggle lies.
Counting Life 20 Days at a Time
Most people measure their lives in years, milestones, and memories. For Jharana Mallick, life is measured in 20-day cycles. Born in a small village, Jharana was diagnosed with Thalassemia Major at a very young age, and her survival depends on regular blood transfusions.
Thalassemia Major is an inherited blood disorder that severely impairs the body's ability to produce healthy haemoglobin, the vital protein that enables red blood cells to carry oxygen throughout the body [1]. To survive, Jharana relies on regular blood transfusions — receiving 600 to 700 ml of blood every 15 to 20 days just to sustain basic organ function.[1,2]
But as Jharana quickly learned growing up, receiving blood brings a whole new layer of quiet anxiety: Is the blood that is keeping me alive actually safe?
The Unspoken Fear Behind Every Transfusion

For patients needing lifelong transfusions, every hospital visit is an exposure to risk. While mandatory routine screening checks donated blood for major infections like HIV, Hepatitis B, and Hepatitis C, a critical challenge remains: the window period.[3,4]
The window period is the early phase of an infection when a donor carries the virus, but standard routine antibody tests cannot detect it yet [3]. During this window, a donor may feel perfectly healthy and pass basic tests, yet the blood can still transmit infections (TTIs) to a recipient.[4]
For someone receiving blood once or twice in a lifetime, this risk might seem small. But for a Thalassemia patient receiving dozens of transfusions every year, that risk compounds over time.
Closing the Window Period with Advanced Screening
To eliminate this hidden threat, advanced diagnostic screening tools like Nucleic Acid Testing (NAT) act as a vital line of defense.[3, 4] Unlike conventional serology tests that detect antibodies produced by the immune system, NAT detects the genetic material (viral RNA/DNA) of the virus directly.[4]
By targeting viral genetic sequences, NAT shortens the window period dramatically — detecting HIV, HCV, and HBV days to weeks earlier than standard antibody testing alone.[3, 4]
Bridging the Access Gap: From Big Metros to Every District
While high-precision NAT screening is standard practice in many developed healthcare systems, its availability across India remains unevenly distributed between tier-1 metro hubs and rural districts.[3, 4]
When Jharana sought blood closer to home, her nearest district blood bank was 200 kilometers away. Even when blood bags were available, advanced screening standards like NAT were out of reach. To secure safe blood and preserve her long-term health, Jharana had to leave her home, family, and village behind to live in Delhi.
A Call for Universal Blood Safety
Today, Jharana is an advocate leading the Sarvesham Mangal Foundation, an NGO dedicated to ensuring access to safe blood for thalassemia patients. She is healthy and thriving because she has consistent access to safe, NAT-tested blood in the capital. Today her voice represents thousands of patients across India who still face regional disparities in blood safety:
