Agenda

Refining early post-discharge management in acute heart failure using an international delphi consensus

This session will present the results of an international Delphi consensus exploring early post-discharge management in patients hospitalised for acute heart failure. Experts will discuss practical strategies for rapid optimisation of guideline-directed medical therapy, the role of biomarker-guided follow-up, and implementation challenges across diverse real-world patient profiles. The discussion aims to provide clinicians with pragmatic frameworks to improve treatment uptake and patient outcomes after discharge.

Date: 11 May 2026 Time: 12:30 - 13:15
Room: Moderated ePoster 4 - Research Gateway

Innovative pathways to accelerate heart failure identification in the community

The symposium will focus on the importance of early diagnosis and management of heart failure. Cardiologists and primary care specialists will discuss diagnostic tools (including POC) and strategies that enable identification of patients at high risk of heart failure in the community as well as accelerated pathways for prompt investigations in symptomatic patients. The audience will learn about different approaches implemented in different countries and new upcoming data.
